🟣 Corynebacterium diphtheriae — The Throat Painter

  • Gram + rods (purple sticks), angular arrangement.
  • Spread by droplets → causes diphtheria.
  • Makes diphtheria toxin (encoded by β-prophage) → blocks protein synthesis (ADP-ribosylation of EF-2).
  • Forms gray-white throat membrane (pseudomembrane).
  • Can damage heart (myocarditis), nerves (neuropathy).
  • Diagnosis: gram + rods with metachromatic granules (red/blue dots), Elek test for toxin.
  • Prevented by diphtheria toxoid vaccine (DTaP).

👉 Like a street artist who paints a thick gray wall in the throat, choking Bodyland, and sending poison to the heart and nerves.

  • Motility: “tumbling” in broth, “rocket tails” via actin polymerization.
  • Causes sepsis & meningitis in newborns, pregnant women, immunocompromised.
  • In healthy → mild gastroenteritis.
  • Treatment: ampicillin.

🟣 Nocardia vs Actinomyces — The Twin Builders

Both are gram + branching filaments (look like fungi) but with opposite habits 👇

Nocardia — The Dust Builder

  • Aerobe, weakly acid-fast.
  • Found in soil, inhaled or enters wounds.
  • Causes lung infections in immunocompromised, brain abscess, skin infections.
  • Treatment: sulfonamides (TMP-SMX).

👉 Like a dusty builder from the soil who sneaks into lungs and brain.

Actinomyces — The Mouth Builder

  • Anaerobe, not acid-fast.
  • Normal flora of mouth, gut, vagina.
  • Causes oral/facial abscesses with sulfur granules.
  • Can spread via sinus tracts, linked with dental trauma, IUD infections.
  • Treatment: penicillin.

👉 Like a builder who lives in the mouth and builds yellow “sulfur bricks.”

Mnemonic: “SNAP” → Sulfonamides = Nocardia, Actinomyces = Penicillin.
