Toxins (baby words)
- Pertussis toxin: turns off Gi → cAMP goes up → lots of mucus & lymphocytes.
- Adenylate cyclase toxin: also raises cAMP (more gunk).
- Tracheal cytotoxin: hurts cilia sweepers.
Stages
- Catarrhal: runny nose, mild cough (very contagious).
- Paroxysmal: cough fits + inspiratory “whoop” ± post-tussive vomiting.
- Convalescent: slow recovery.
Prevent: Tdap/DTaP vaccine. Treat: macrolide (eg, azithro). If allergic → TMP-SMX.
🥛 Brucella — “Cow-Milk Sneaker”

Comes from unpasteurized milk or animal contact. Hides inside macrophage trucks and rides to the spleen/liver, making caseating granuloma forts.

Lives in warm water (AC, hot tubs). You breathe the mist, not from other people. In lab it likes charcoal yeast extract with iron & cysteine. Best seen with silver stain.
Diseases
- Legionnaires’ disease: severe pneumonia (often one lobe), GI and CNS symptoms, labs with hyponatremia.
- Pontiac fever: mild flu-like.
Dx: urine antigen test. Tx: macrolide or fluoroquinolone.
🧪 Pseudomonas aeruginosa — “Blue-Green Pool Painter”

Loves water & hospital gear. Smells like grapes, paints wounds blue-green (pigments pyocyanin/pyoverdine). Makes sticky biofilm and a mucoid capsule in cystic fibrosis lungs.
